1.Incision

lateral decubitus position

Longitudinal incision over the sinus tarsal used by #11mess

peroneal tendons 를 tenotomy scs로 identified하고 senn vein 으로 retration

fleer와 cobb's로 tongus fragment를 expose한다.

 

2.Fixed with interfragmentary screw

fracture fragmentary site에 irrigation (30cc sry)

fragment에 k-wire#2 으로 temporary fixation 한 후 같은 길이 k-wire로 길이 확인 하고

k-wire를 제거 한 후 Compact hand 2.4mm 로 interfragmentary screw 로 fixation 한다.

 

3.Reduced tuberosity fragment

heel 부분에 k-wire#3 으로 temporary fixation 하고 같은 길이 k-wire로 길이 확인 (g-pin 역할)

11mess로 incision 넣고 mosquito로 opening한 후

 

 

1)4.5 HCS Set내에 3.2 Reamer로 drilling 한 후

2)screw head predrilling

3)4.5 HCS Screw 를 insertion 한다.

(posterior heel, inferior heel부분에 compression screw)

 

 

 

4.close

irrigiation 후 100m JP Drain 넣고

wound close : 3/0 vicryl, 4/0 nylon
