Femoro popliteal (above knee) by pass 대퇴슬와동맥우회술

하지폐쇄성 동맥경화증(vascular sclerosis)에 대해 슬와동맥(오금동맥, popliteal artery)에서 대퇴동맥(넙다리동맥, femoral artery)까지 우회로를 형성하는 수술

 

 

 

 

TS Minor set + Angio set + puspan set + Thoracotomy drape

prn) vascular clmp (1자 clmap 사용시)

 

<부속>

hemoclip 6개 (주로 small size 사용) 

pott's scs 3개 (pott's scs -> reverse scs)

mastoid 2개 (큰것: popliteal, 작은것 : femoral)

 

<수술준비>

heparin solution

Tissel

주황색 tunnel + tunneler + U tape 

*arteriotomy 하고 난 후 excessicve calcifiaction 이 있을 경 우

(3Fr Fogarty catheter + 3way + 1cc)

 

 

 

 

 

 

1.Common Femoral artery - Incision & approach

1)Common femoral artery 는 pubic tubercle 과 the anterior superior iliac 사이 : icision 10mess 

2)soft tissue 를 med debakey 와 Bovie 로 femoral sheath level 까지 approach 
-with Mastoid ret (+army) providing adequate exposure.

 

 

 
3)proximally - inguinal ligament.
Distally - proximal superficial femoral artery.

4)profunda femoris artery is identified and exposed.
처음에는 mixter 로 sling을 걸 때는 blunt, right-angled clamp 로 u-tape(1/2) sling + mosquito

*만약, nondiseased 한 superficial femoral artery와 profunda femoris artery 의 branch 들은

Sling + mosquito 을 걸어 놓거나 Hem-o-clip (small) 로 정리 

 

 

 

 

 

 

 

2.above-knee popliteal - Incision & approach

 

 

1)A longitudinal incision is carried out with 10mess allowing anterior to the sartorius muscle above the knee
->soft tissue is divided with bovie allowing the sartorius muscle to be retracted posterolaterally.

->deep fascia overlying the adductor canal is then visible. 

->fascia is divided sharply with Metzenbaum scissors, exposing the popliteal fossa.

->popliteal artery is dissected from the surrounding fatty tissue.

 

2)The artery is carefully mobilized from the popliteal artery 

to ensure that adequate length is available for the anastomosis. 

: 처음에는 mixter 로 sling을 걸 때는 blunt, right-angled clamp 로 u-tape(1/2) sling + mosquito

: brnach 는 hemoclip 으로 정리, sling + mosquito

 

 

 

 

 

 

 

 

3.Provimal and distal arterial tagerts - preparation 

 

With Orange Tunnel + Tunneler 

-> tunneler 끝에 u-tape 통과하여 popliteal과 femoral 에 통과한 u-tape + mosquito로 물어놓는다.

-> tunneler 를 제거 한후 graft open 하여 u-tape을 따라 통과

ring이 달려 있기 때문에 suture 할 부분에 ring은 분리하여 막가위로 제거

->제거 한 후 anastomosis 시작한다.

 

 

 

 

 

 

 

4.anastomosis - Femoral + popliteal 

*arterial clamp 잡기 전에 Heparin 주고 5분 후 anastomosis 시행

 

 

 

angio set 내에 vascular clamp 종류 7가지

femoral artery : ㄱ가 ret 4개 전부사용, artery 와 graft

popliteal artery : santisky clamp 1개 

 

ㄱ자 vascular clamp(2ea)로 Common Femoral Artery clamp 하고

vascular 끝도 clamp(1ea)로 clamping

angio set 내에 vascular clamp ㄱ자 형태 4개 다 사용

Femopral: arteriotomy is created with a no.11 mess on the anterior wall of the Common Femoral Artery 

->extended proximally and distally with potts scissor to a size appropriate for the grafty

->7/0 prolene 으로 graft와 artery 간 anastomosis 

*angio set 내에 micro needle holder + micro fcp

 

->첫번째 실 suture 한 후 실모기 물어 놓고 running suture 

->첫번째 실 suture 다 하면 실모기 풀고 두번째 실로 suture

->suture 다 하고 나면 nerve hook으로 tight하게 suture

->tie후 neelde cut  

->reinfoce suture :  7/0 prolene + soft pledget

 

 

 

 

 

 

 

5.close

irrigation 한 후 bleeding control : surgicel, tissel 2ml 

reinfocre suture : 7/0 prolene + soft pledget
