1.position

beach-chair position

(mayo table 1개 더 준비)

 

2.incision & approach

skin incison used by 20mess

metz으로 deltoid와 pectoral muscle 사이에 approach  

(deltopectoral incision보다 deltoid splitting approach 를 주로 선호 axillary nerve 손상 피할수 있다.)

Mastoid, gelfi retractor, sholuder retractor로 retraction 

proximal humerus 의 defect 한 부분을 cobb's 로 expose한다.

 

3.reduce fracture

internal rotation 하면서 supraspinatus tendon 에 ethibond no.2 로 suture 해 놓는다. 

hohmann 으로 humerus head와 shaft를 노출 시킨 후

k-wire로 fracture fragment를 reduce

 

4.position plate on bone

proximal humerus plate를 apply 

humerus head 부분에는 plate에 insertion guide를 attach  

3.5 locking screw (2.8 drill bit) - depth - power driver - torque driver 

 

5.close

rotator cuff tears suture : no.2 ethibond 

suture: vicryl 1/0 - 2/0

skin : derma-bond

drain : 400M
