1.position

lateral decubitus position 

 

2.incision&approach

20mess 로 skin incision 

bovie로 subcutaneous tissue 를 dissection. mastoid 로 retraction 

blunt하게 gluteus maximus 를 split 한다. 

external rotator exposure하고 hip self retractor로 retraction 하고

posterior capsurle에 ethibond no.2 로 tagging suture + mosquito

 

2.femur neck osteotomy

hip capsule과 gluteal muscles 사이에 sharp homan으로 retraction

cutting guide로 cutting 정도 확인

reciprocating saw로 femoral neck cutting 

(st.#5 osteotome으로 cutting)

tower clip으로 head 제거 

(제거 안 될 경우 cork screw)

post operative flextion 에 방해되는 osteophyte제거 : ronger + mess 10 (ㄱ자 hip ret)

 

3.Acetabular sizing

cutting 한 femoral neck으로 size 확인 후 

trial bipolar shell 확인 후 implant open

 

4.Femoral canal broaching

anterior-posterior ret (수술 부위의 반대쪽), sharp homan으로 

fcp과 bovie로 osteophyte 정리 

box osteotome

start reamer 

rasp 

(12~15도 anterversion 각도)

 

5.femoral canal preparation 

canal에 persa-vac irrigation으로 irrigation 하고 

epi gauze로 canal에 bleeding control하고 

cement plug insertion

vaccum mixing system 으로 cement mixing하여

cement gun으로 cement insertion 

stem insertion (stem+distal centralizer + proxiaml centralizer)

stem pusher + cement pusher

cement 확인하고 

femur head assemble + head pusher 로 impator 

 

6.closure

bleeding control & irrigation

greater trochanter에 2.0 drill 구멍낸 후에 ethibond no.2 + 직침으로 suture 

drain : 800 L Hemovac 

gluteus maximus suture : no.2 vicryl 

deep fascial : no.2 vicryl 

subcu : 2/0 vicryl 

skin : stapler
