Traiangular FibroCartilgae Complex (TFCC)

-Stabilizing the distal radioulnar joint (DRUJ) during forearm rotation

3.준비물품

11,15mess

4*8gz, 4*4gz

bipolar + line

JP 100M

Suture materail : 3/0 vicryl, 4/0 nylon, 3/0 nylon, pds 2/0 

4.0 drill bit

*4.5 HCS Reamer만 사용 (3.2 reamer)

 

4.anchor

arhtrex push-loc 

 

 

 

 

 

 

*수술 전 Wirst traction 설치 

 

1.camera port incision

wirst joint 안에 n/s 을 채운다 (30cc sry)

11mess incision

st.mosquito로 opening

scope insertion

 

2.working port incision

18g needle로 joint level확인 후 

11mess incision 후 st.moquito로 opening하고 

TFCC 의 손상 유무를 Probe로 확인 후 debrider로 정리 

 

3.mini-open (skin incision in the ulnar side)

marker로 design 한 후 

11mess로 incision 넣은 후 

senn-vein으로 retraction 

TFCC Guide를 넣어 drill hole을 만들기 위해 k-wire #2 으로 guide marking 한 후 

4.5 HCS Reamer (3.2 reamer) 로 reaming한다.

4.0 drill bit로 suture 가 들어갈수 있도록 길을 만들어준다.

 

4.inside-out suture technique

mini open한 곳에 18g spinal needle을 넣어

18g needle 구멍으로 arthrex double arm needle 2/0 fiber wire(needle cut)만 통과 시킨다 x2

fiber wire는 mosuqito 로 tagging 해놓음.

PDS 2/0를 needle잘라서 suture lasso 역할로 fiber wire를 다시 mini-open한 곳으로 빼낸 후 

anchor가 들어갈 자리에 mini-anchor drill로 dril hole 만들어준 후

fiber wire를 tight하게 당겨서 push-loc(2.5x8mm) 로 anchor 박아준다.

 

 

*push-loc 사용법

fiber wire를 push loc에 mosuito로 통과시켜서 tight 하게 조인 후 

anchor를 hammer로 넣은 후 scs로 cutting
