Lapa Uretero Neo Cystostomy (Lapa UNC)

-적응증: 방광암 (bladder cancer)

 

 

1. set	URO minor set laparotomy package + 발자루  소방10
2. 부속	endo set, endo needle holder 2ea
3. 준비물	golve 무영등 Mess 15 Bovie blunt Marker & 자 Veress needle + 3way + 10cc sry terumo 0.035 st D-J stent (원하시는 size) X-ray 4*4 4*8gz 2pack Enema sry 2ea sani-sleeve   *dressing sorbact 5*7    *Suture material 4/0 vicryl 5ea (15cm정도 잘라 준비) 2/0 j type needle (cutter tomson 사용) 3/0 nylon  prn) V-LOC 15cm *trocar 11mm exel trocar 2ea(청록색) 5mm exel trocar 2ea(청록색) *lapa instrument CO2 line light cable 10mm 0' scope Hemo-o-loc ML/L (초록/보라)  보온병 Harmonic Handpiece & line Ethicon hand bovie  Instrument pocke TUR-Y line *cysto instrument 22Fr sheath  long bridge  TUR-Line 4mm 30도 scope urethra catheter

 

 

* Foley는 수술안에서 준비 : Elastomer 16Fr

 

 

 

 

 

 

  

 

 

 

1) Lithotomy position 

​Lt ureter : surgeon은 환자의 Lt에서 시작

 

2)Trocar 

camera port : 11mm EXCEL port 

main port : 11mm Excel port

assist port : 5mm Excel porx *2ea 

 

15번 mess incision -> mosquito 로 open -> tower clip으로 fascia 들어올리고 -> veress needle + 3way + 10cc연결

 

3) sigmoid colon mobilization (하기 때문에 enema tube, L-tube 를 수술 전 넣기) 

olympus  fcp 으로 bowl을 잡고  ethicon bovie 로 sigmoid colon 을 moblization 

dissector, ethicon kelly 

 

4) ureter mobilization 

olympus fcp, ethicon kelly, dissector로 ureter mobilization 

-> u-tape sling 8cm + hemolok ML(초록색)

-> Ethicon hand boive로 mobilization 

-> ureter의 stricture 된 부분 확인 한 후 hemoloc L(보라색)으로 Clip 한 후 cut scs로 cut 

 

 

 

 

 

4) neoorifice 

bladder dome에 neoorifice를 hand boive로 내고 scs, ethicon kelly로 opening

ureter와 bladder를  4/0 vicryl (20cm) 으로 interrupt suture 

(sympubis 쪽 5mm port 추가 삽입해 실을 metal endo needle holder로 잡아 tension 유지) 

 

 

 

*interrupt suture 전에 bladder filling : 250cc - foley 를 통해 

*cystoscope 보면서 D-J insertion 

 

peritoneum 일부를 v-loc 15cm 으로 repair

leakage test 150cc

 

 

5)close

2/0 J needle로 11mm trocar부분 suture

JP 100L 

3/0 nylon suture
