IVOR-LEWIS op (Open Laparotomy + OpenThoracotomy)

-적응증 : 식도암 esophageal cancer 

-식도를 절제하는 방법 : 개흉술 + 개복술 + 식도절제술

 

 

 

 

 

 

Ⅰ.준비물품

​

 

set	TS Thoracotomy set GS SPINAL SET 만약 사용 못할 경우 TS Omni retractor set	GS Major set GS SPINAL SET
부속	​ TS Ret set TS LOBECTOMY SET andrew suction tip open 용 duval lung fcp	Open 용 hem-o-lok (주로 초록,보라만 사용) wound protector 초록색 가장 큰것
준비물품	s-pen​ 10번 mess 3M nelatone 7 + 3way : jejunostomy deaver cover 2 자석판 bovie short tip / medium tip(주로사용) / long tip 수세미 spoid 2 4*8 pad : spleen protector *suture -vicry 4/0 -black silk tie용 긴거 3.4.5호 -blakc silk CR 3/0 -Polysorb 1/0 continuous *close -pds 1/0 -polysorb CR 1/0 -Nylon 2/0 -stapler	10번 mess bovie s-drain 8번 : esophagus retraction nelatone 5번 : bovie protector *suture black silk 3/0 CR (Lg) - esophagus 고정용 prolene 2/0 - 직침 or round black silk 3/0 continuous *close polysorb 1/0 CR Vicryl 1/0 (2ea) vicryl 2/0 (1ea) vicryl 3/0 (1ea)

 

 

 

 

 

 

Ⅱ.procedure

 

 

 

 

 

 

 

 

<laparotomy>

 

1.upper midline incision

​10mess incision

 -> bovie 로 abd division

->wound protector 大 하고 소방을 위아래로 댄다.

->spinal ret  (mayo Type) 

 

 

2.gastric tube preservation along gastroepiploic a

​short gastric & splenic vessel division

​pf.문 or pf.성 - long fp

pf.안 - debakey fcp

spleen옆에 wet pad로 protect 한다. (ring이 pad안으로 잘 가둬지게)

​open용 hem-o-lok 초록색, 보라색

artey는  long rt angle으로 dissection한후  -> hem-o-lok(green/sm) -> harmonic 으로 cutting

​

Lesser curverture dissection with gastric & division : bovie medium tip, harmonic

hillar dissection with esophageal dissection : bovie medium tip, harmonic

 

 

 

 

 

*spinal retractor set

 

 

 

spinal retractor set는 처음에 mayo tupe

깊어질수록 deaver type과 richardson 함께 사용

liver쪽에는 hemilton type retractor 사용

3.hillar crus division

gastric tube macking

 

 

with TLC New titanum 75 * 3

jaw를 확실히 열은후 장착 cartalige는 kelly로 out

blue 단계

*cutting 할 때는 L-tube를 올리고 cutting 해야 한다.​

seromuscular suture (reinforce ) : 3-0 blacksilk CR sutrue

bleeding control

4.Feeding Jejunostomy

4/0 vicryl 로 purse-string suture

->11mess puncture

->st mosquito or mosquito opening

->nelatone 7번 넣은 후

->abdomen skin 15mess

->st mosquito or mosquito opening

->nelatone을 skin밖으로 빼낸 후 jejuno와 abdomen 을 black silk 3/0 CR *4ea tagging

->skin 밖은 3/0 nylon tagging

​

 

 

 

 

 

 

5.close

먼저 PDS 1/0 continuous 로 perionium

1/0 CR  로 fascia

skin은 nylon 으로 2/0 로 닫고 medress로 붙인다.

drape을 걷은 후 medress위에 4*4를 덮고 3m 조각으로 붙여 둔다.

 

 

 

 

 

 

<open thoracotomy>

 

1.Rt. posterolateral thoracotomy incision

10mess -> Bovie disseciton + tooth

army, baby retraction

 

LD Muscle resection -> SA Muscle saving

Rib 이 보이면 Scapular ret 걸고 rib count하고 dissection 한후 rib을 자른다.

-오른쪽 rib이면 왼쪽 guard가 있는 cutter를 사용 한다.

 

ret set내의 큰 chest ret와 ribbon ret걸고

bovie 20으로 낮춘 후 부속lung fcp사용

 

 

 

 

 

 

2.Azygus vein dissecition & division

Lg La rt angle과 Lg Lg debaky 를 이용 하여 azygus vein을 diseciton 한 후

5호 silk tie 한후 mosq로 tagging (suture tie cut안함)

 

 

 

 

 

 

3.thoracic duct dissection & division

with hem-o-lok (ML)

Esophagus dissection

bovie lg tip으로 dissection 하여 penrose drain #8 걸어 lg lg kelly 고정

(Lg rt angle에 8번 perose clamp)

박리중 보이는 branch vessel은 med clip과 bovie를 이용하여 처리

 

Mediastinal L/N Dissection

lg trt angle 과 bovie를 사용

 

 

 

 

 

 

 

4.Longitudinal esophagotomy

Lg cutting scs로 esophagus르 ㄹ반 정도 자르고 betadine으로 닦은 후 bovie로 나머지 자른다

stump는 Lg Lg kelly로 잡아 둔다.

(위 부분은 4*4 로 한장 덮어 둔다)

 

Gastric tube delivery

Allis로 잡아 올린 후

EEA Set내의 clamp집은 후 2-0 prolene 직침 통과 : purse-string suture at proximal

EEA 25 + Envil clamp (EEA Set)

 

esophagectomy below envil gastrosotmy

stomach에 bovie로 구멍 뚫은 후

betaine으로 닦고 EEA Body 넣고 fire

Rt angle로 fire여부 확인

body 나올 때 소방 대기

tagging 했던 prolene tie후 cut

3/0 CR (Lg Lg needle holder 부속) serosa suture 2-3번 나눠서

중간중간 suction 하고 L-tube insertion

 

 

 

 

 

 

 

5.Distal gastric tube resection

with TA 90-4.8로 Stapling

11번 mess로 cut

suction 한번 더하고 out

staple찍힌 stomach에 3/0 continuous b-silk로 serosa suture

3/0 CR로 Reinforce with 3/0 proelen (한쪽 needle cut, continuous suture) stomach를 tube 모양으로 만든다.

 

 

 

 

 

6.stomach tube fixation to mediastinum

with 3/0 black silk CR (lg lg needle holder)

Bleeding control & Irrigation

 

 

 

 

 

7.C-tube insertion

- 2/0 nylon retainging sutre 하고 mosq로 cut

st 집어 넣어 thorax 안쪽에서 tube를 물어 밖으로 빼낸다.

tube 의 hole이 부족하다고 생각하면 moq로 잡고 추가 hole만들어 insertion

​chest tube 28fr insertion 

1)mess 15 incision -> bovie -> mosquito -> kelly 로 opening

2)nylon 2/0 tagging suture

3)chest tube insertion 

 

 

 

 

8.close

long pin 과 tooth를 사용

1)pericostal : vicryl no.1 - mosquito tagging 해 두었다가 intercoastal이 끝나면 suture

2)intercostal: polysorb no.1 (CR) 5ea + 각각 mosquito tagging 

3)muscle : vicryl 1/0 (2ea)

4)fascia: vicryl 2/0 (1ea)

5)subcu: vicryl 3/0 (1ea)

6)skin: stapler

7)dressing: 25cm, tube site 4*4 2장 접은 y, 2장 접은 뚜껑

 

*close suture material 

1)pericostal: vicryl no.1 

2)intercoastal : polysorb으로 5ea suture 후 mosquito tagging - 닫을때 approximater 사용 (Ret set 내)

3)muscle, fascia, subcu : vicryl 1/0 suture

4)skin: stapler
