tibia shaft fracture - Stryker T2 Nail

-적응증: 경비골 골절

 

 

 

 

 

 

 

paratendenous incision is made from the patella extending down approximately 1.5–4cm in preparation of nail insertion.

The Patellar Tendon may be retracted laterally or split at the junction of the medial third,

and lateral two-thirds of the Patellar Ligament. This determines the entry point

 

 

 

design #20 mess incision

-> criel  dissection

-> Ret : mastoid (spreader)

 

 

 

 

entry point 에 awl + 망치 (awl은 구부러진것 사용)

-> g-pin(처음에는 ball type없는걸로) + handle insertion(사진에서 가장 왼쪽)

-> 회사 망치 사용

-> c-arm 으로 g-pin 이 제대로 insertion 되었는지 확인 후

-> 동일한 g-pin 으로 nail 길이 확인 or 회사용 depth로 nail 길이 확인

 

reaming 8부터 시작 9.10.11 은 끝에 reaming head변경

-> reaming 시에는  ball type guide로 변경

-> 1mm over-reaming

-> reaming 시에 reaming head가 떨어지지 않게 주의 하고, 1st assist 는 g-pin pusher로 guide pin 이 빠지지 않게,

2nd assist는 kelly로 잡아줌

 

nail insertion

처음에는 aiming arm없이 insertion

 

 

-> aiming arm 을 설치

-> targeting sleeve 넣은 후 15mess incision -> criel dissection -> drilling -> depth(회색 긴) -> screw

-> proximal screw모두 다 넣었으면 arm 제거 (universal driver)

-> end cap

 

disital 쪽으로 내려와서 c-arm check

A/P, Lateral 동시에 check

-> marker로 hole 위치에 선을 긋고 표시

-> incision 15mess

-> criel disseciton

-> target pin ( 없으면, s-pin 3번으로 헤라클레스로 잘라서 insertion 하기도 함) + 망치로 넣은후

c-arm 찍어서 확인함, 제거시에는 cutter와 망치로 제거

-> free hand drill bit (짧은 drill bit) + 망치

-> 동일한 drill bit 와 비교하여 길이 확인

-> empty 로 chuck 에 문 후 drilling (2nd assist가 key로 물어줌)

-> depth 생략한 후 screw insertion

-> c-arm 으로 마지막 확인

end cap 과 모든 screw 의 driver는 동일 하다.
