​Anatomy

​

​

​적응증: pituitary adenoma 뇌하수체 선종

-호르몬 비분비성 뇌하수체 선종은 수술적 제거가 우선

-호르몬 분비성 뇌하수체 선종 : 약물 치료 + 수술적 제거가 병용되는 경우가 많음

-프로락틴 분비성 뇌하수체 선종 : 약물 치료가 우선적으로 선택

Procedure

 

 

 

 

 

1.supine position

​

2.draping
Utility - registration facial mask 보이도록 얼굴은 3M 으로 고정

​

3.approach

otocaine injection 2@
external rhinoplasty 접근 방법으로 V-shape 으로 #15 meas incision

Adsob tooth , iris sca로 mucosal incision posterior septum at the sphenoid level 까지 

septum 사이로 opening
Tagging suture : 5호silk + round needle 
Septum 을 sasaki scs 로 remove (recon시 사용)

 

4.sphenoidal sinus 의 anterior wall bone을 들어내고 sphenoidal sinus 제거 한 후

 

sphenoidal sinus post wall open 한다.  -endoscopic drill, burr 
hardy ret 大 apply 한 후

punch, gauge, 2mm osteotoe, hammer, sasaki cutting과 scs

bone fcp, nasal fcp

 

5. navigation 을 통해 위치 확인

carvenous carotid artery 도 확인

확인후 micro scope in

 

 

6..dura opening & Tumor removal

dura open 시 위아래 15mess 로 incision

 

 

ring currette, dissector, pituatary fcp, st scs & cvd scs, suction 으로 debalking

모든 기구는 주로 얇은것을 사용한다. 작은것에서 큰것으로 변경

 

6.skull base reconstruction and cerebrospinal fluid leak repair

tumor 가 제거 되고 나면 bleeding control : surgicel, cotton (nasal에 들어가는 크기)

CSF Leak 되는지 확인 하고

만약에 CSF Leak 가 되면

 

Medium-sized CSF leaks (grade 2) or grade 1 leaks with a large amount of intrasellar dead space

require the placement of an abdominal fat graft을 시행한다.

수술 후 lumbar drain 시행

 

7. closure

sellar reconstruction : 그전에 보관한 septal bone 을 채워준다.

-환자가 이전에 수술을 받은 경우 implant MEPOR (TSI)를 사용한다.

septum closure 6/0 catgut

6/0 Nylon skin suture
