Percutaneous Endoscopic lumbac Discectomy​

​PELD(Percutaneous endoscopic lumbar discectomy) 내시경 레이저 디스크 제거술, 내시경 디스크 수술

요추 척추관 협착증이 동반된 추간판 탈출 환자에 대해서는 내시경 수술은 수술중 신경 손상 위험성이 높아 수술 금기증

-> 만약 주 증상이 요추 척추관 협착증이 아닌 추간판 탈출에 의한 외측함요에서 신경근 압박으로 발생한 경우

경피적 내시경을 이용한 trnsforamnial approach 접근하여 신경근 감압하는 방법

-> 매우 효과적인 최소 침습법 수술이 될 수 도 있다.

Proceudure

​

1.foley insertion + perineal cover

​

2.prone position : spine table

 

3.skin preparation

​

4.C-arm guide 하에 A/P Lateral view를 check 하여 Spine body level확인 한후 marker

​

​

5.C-arm guide 하에 endoscopoc sheath 를 이용하여 ap view 에서 interlamina space를 확인함

->spinal needle(endo set내의) 으로 entry point로 잡고

->stylet 제거하고 guide pin 삽입

->11mess incision

->mosq 로 넓힌 후

->첫번재 dilation troca를 넣고 g-pin 을 제거 한다.

->diameter를 늘려가며 endoscopic diamter를 늘려가며 endoscopic sheat 를 insertion

->최종 troca 3개 중에 operatior가 한개를 선택하여 넣고, dilation troca를 제거 한다. 주로 물결무늬 troca사용

->troca에 locking connctor를 연결한다.

->silicon잘라 놓은 것 주에 길이 선택하여 scope에 끼운 후 connector에 넣는다.

6.interlamina space를 통해 삽입된 endoscope를 통해 vertebral canal space에서 extrusion 되어 있는 disc 확인

margin of pedicle 에는 soft tissue와 fat, blood vessels로 둘러 쌓여져 있는데,

->indigocamin으로 disc를 표지하고 spinal needle 꼽아서 사용

->표지된 disc를 jocker,blade dissection punch 와 pituatary 로 제거

(pitu는 거의 제일 작은것 금색 손잡이만 사용)

->bleeding control은 bipolar를 이용하여 hemostasis시행함

​

Guide wire - dilator 해서 사용 하는 channel system은 ( YESS System ) transforminal 를 통해 disc space 로 안전하게 working zone으로 통과 할 수 있게 한다.
