Endo DCR

 

-silicone tube intubation : 부분 폐쇄

-endo DCR : 완전 폐쇄

 

*차이 : 실리콘관의 주행 및 매듭의 위치 차이

-silicone tube intubation : 하비도의 코눈물관의 출구 쪽에서 매듭이 만들어짐

-endo DCR : lacminal sac에서 비강으로 바로 이어지는 새로운 출구에 매듭이 만들어짐

2. 준비물품

준비 물품	･ Glove ･ 4x4 Gz ･ OT 면봉 ･ Pana pad 3ea ↑ : 알콜 ･ ENT packing Gz. 3pack ↑ ･ Mess #12, 15T ･ Keratome blade (2.75mm) ･ 10cc, 5cc, 2cc 1ea씩 ･ 50cc syringe 1ea ･ 25G spinal ndl. ･ Marking pen ･ Puspan 1ea ･ Sani sleeve ･ 수액 set + extension
suture	･ 6-0 b/s (tube tie, skin) ･ 5-0 prolene (CIS tube 고정)
Implant	･ CIS tube (0.94 / 64C / 굵은 것, 0.64 / 64 / 얇은 것)
약물	･ 1% LDC + 0.2cc Epinephrine ･ 4% LDC + bosmin 20cc (50cc syr.) ･ Anti-fog ･ GM (Irrigation용 : N/S 38cc+GM ２cc Mix) ⇒ 50cc syr. ndl. 구부려줌) ･ Effexin ･ Nasopore Forte (8cm)
PRN	･ 2cc syr + healon ndl. ･ MMC 0.02% + 긴 면봉 (2분 apply) ･ Trephine

 

 

 

 

 

1.수술 준비 및 skin pre

1)Induction 후 Hand table에 DCR Kit 를 사용하여 ENT packing Gz, pharyngeal packing 한다.

(nasal speculum 1ea, bayonet fcp 1ea, concotomy sc 로 콧털 정리 후 4% LDC + bosmin에 적신 ENT packing Gz.)

2)skin pre: 0.05% hibitane + Sponge fcp 

 

 

2.Local injection

1% LDC + 0.2 epi = 2cc syr. + 25G spinal ndl.

 

 

3.Turbinectomy

nasal mucosa를 freer, sickle knife (or keratome)으로 incise하고

cvd. curette(소), Freer 로 lacrimal bone을 찾는다. 

debrider 2.7 st 또는 cvd 로 setting

 

<ESS Set에서 주로 사용하는 기구>

-cup fcp (소) : st, upward 

-cutting fcp (소) : st, up 사용

-kerrison punch 위, 아래

-cvd cup fcp

-back biting

-curette 소

-S-tip (얇은 것 → 굵은 것) , frontal s-tip이나 maxillary s-tip은 잘 사용안함.

 

 

4. Lacrimal bone drilling

bone opening 을 Back biting, kerrison punch로 bone opening을 만든다.

diamond, cutting burr 로 lacrimal bone drilling 한다. 

 

5.Lacrimal probe passed via inferior canallculus into lacrimal sac

puncture dilator, Lacimal probe 0,00 (silicone intubation set)를 sac에 넣어본다.

(통과가 잘 안되면 trephine으로 뚫고, 2cc (N/S) + healon ndl. 사용해서 통과한다)

inferior canaliculus을 통해 lacrimal sac으로 illimi insertion 

-bishop, dilator, 면봉으로 lacrimal duct를 뚫어 DORC illumi를 끼우고 illumi가 완전히 들어가면 light ON 해준다.

-illumi light를 보고 lacrimal bone을 더 노출시킬지 보고 결정. 

 

 

5. CIS tube insertion

미리 size 선택하여 GM solution에 silicon 부분 담가놓은 후 

CIS tube를 puncture해 sac으로 나오게 하여 st. mosq로 tube를 코로 나오게 한다.

→6-0 Black silk를 tube와 함께 tie한다. (1/2 cut, ndl cut ⇒ 바늘 없이 tie용으로 사용)

→tube는 두꺼운 needle holder로 잡고 black silk로 tie한다.

→실 자를 때는 st.iris scs로 자른다. 

→5-0 prolene (st. N/H)으로 lateral nasal wall에 tube를 고정한다.

 

 

6.Dressing

Naso pore Forte (+연고) apply
