-injection 준비 : 1% lido+ epi 0.1cc/

-heparin sol 준비 :n/s 1L bottle + heparin 1cc

-4% lidocaine 준비

 

 

 

 

 

 

1.Tracheostomy

laparotomy drap 하고 난 후 hypopharyngectomy 할 때는 thoracotomy drap으로 다시 시행

t-cannula 대신해서 reinforce tube 넣어 마취 한다. 

 

2.hypopharyngectomy

1)Skin pre

skin pre시 oral betadine irrigation도 함께 시행

 

2)Incision

10mess mess -> bovie, bipolar로 dissection

2/0 black silk CR 로 tagging + mosquito clamp 하고

깊어지면 갈고리로 tagging

3)L/N dissection

4*4gz 사용

sharp mosquito, sharp metz으로 dissection

clip S/M (black silk 2/0) 으로 Vessel Ligation 하고 harmonic으로 cut 

Lymph node 2,3,4

4)hypopharyngectomy

allis로 hypopharyx잡고 bovie, bipolar, harmonic

sharp mosquito, sharo metz으로 박리 

thyrodie cartilage bone cutter로 자른다. (oscallating saw도 사용)

hypopharyx가 나오면 irrigation

 

3.ALTFF 

hypopharyx의 defect만큼 ALT에서 flap 조직을 땐 후 Graft

4/0 vicryl로 flap을 고정 한 후

micro로 보면서 nerve, vessel은 9/0 ethilion suture

vessel dilation : 4% Lidocaine, heparin sry +healon needle 

bleeding control : surgicel, tissel 4ml 

 

4.Close 

ALT close: 4/0 vicryl - skin stapler - JP100M

Hypopharyngectomy close : 4/0 vicryl - 4/0 nylon - Hemovac 400M
