GV+ 26G short needle로 marking

local inj. mess#15, adson tooth + metz, ragnell or senn-vein, #15으로 연골 자르고 꺼냄

Vicryl 5-0, Ethilon 5-0

betadine sol, hygiene

4%LDC+Bosmin, 1%LDC+0.1cc epinephrine, N/S, 연고

small cotton ball 3-4개 준비

 

 

 

1.Skin-pre

5% betadine solution + sponge fcp

 

2.코 털 제거(+ 연고)

set 내 iris cvd. + 단비경-> 불결로 취급.

irrigation : 50cc syr + N/S or betadine.

 

3.Local injection

1)10cc + 26G Lg. needle -> 1% Lido + 0.1 epi inj 후 5분 Waiting

2)Nasal packing: Rhino 접시에 실솜 6장 , bayonet fcp, 단비경

 

4.incision

bovie 12-12-12

#15 mess → angled sci (=converse sci.) & tenotomy sci 로 dissection → Freer

Ret : 2-prong ret. 좁은거/ skin hook/ Ragnell / aufrich / senn-vein /blunt 2-prong ret.

fcp : adson tooth / castro / multi tooth(catiliage fcp)

 

5.Cartilage Remover

(Cartilage는 반드시 깨끗한 N/S 종지에 담궈놓기)

septum 을 josep septal knife로 open 하고

Septal knife 와 fleer, 하키모양 septal knife로 로 catilage 를 떼어 낸다.

*osteotome: Rubin 사용 → 11번 mess incision후 osteome

6.implant insertion

cartilage 제거 한 후 코 높이를 종이자로 재서 잘라서 septum에 넣어서 angle 확인 후

catilage를 종이자에 맞춰서 잘라서 사용한다. 

*megaderm 사용 시 N/S 담궈놓기.

Vicryl 4/0 로 Catilage와 megaderm을 겹쳐서 suture

catilage를 septum에 넣고 임시 고정을 위해 23g needle로 잡고 PDS 5-0(round W9201H)로 suture

nasal tip은 남은 Cartilage를 crucher로 사용 후 tip 올려준다.

 

7.closure suture

vicryl 4/0 -> vicryl 6/0 -> skin은 nylon 7/0

 

8.close

Silastic sheet -> merocel 반 발라서 사용 -> betadine soaking -> aqua splint
