VATS hyperhidrosis 

다한증 (hyperhidrosis) 

 

<indication>

2016/08/26

Excessive sweating (hyperhidrosis) can now be treated with a minimally invasive operation

via Video Assissted Thoracoscopic Symphatectomy (VATS).

 

교감신경계는 자율신경계의 한 부분으로 평활근과 심근, 땀샘을 포함한 다양한 분비선에 대한 신경 지배를 담당

다한증에서 교감신경 수술이 효과가 영구적이나 부작용으로 <보상성 다한증>

 

 

 

 

 

 

수술준비

​TS Hyper set + new package + pither or 보온병 (warm n/s 으로 camera 의 anti fog역할)

2mm scope + light cable + sani sleeve

무영등, 11mess, 4*8gz, 10cc srynge

endo bovie line + minishears

mini port 3ea + CO2 line

guide wire + 노란색 central line

 

 

 

 

 

 

 

procedure / 수술과정

 

1.supine position 후 마취 후 semi-sitting postion 으로 변경

 

2.Drape : utility 사용 안하고, 소방4 -> usual drape

 

3.Intercoastal block : Bupivacaine injection 후 mess 11번으로 nipple 아래 incision 후 miniport 로 chest wall 뚫고 2mm scope insertion

->무영등 계속 해서 on

 

4.miniport port에 CO2 Line 연결 ( ㅜ botton 후 high On : insufflation flow 10 )

 

5.Axillary 에 incision 한 후 miniport로 chest wall 뚫는다 - 이 때 port의 clamping 해놓은 상태

6.Rib count 하여 Hyper set내에 있는 긴 probe로 dissection 하여

endo bovie line 연결 한 minishears insertion 하여 ( coagulation 20 ) 하여 sympathicotomy 를 한다.

다한증 나는 부위에 따라 T2, T3, T4 결정하여 sympathicotomy 시행

->heart rate가 떨어지면 sympathicotomy 성공

 

7.sympathicotomy 시행 후 CO2 배출

->Mini port안에 wire (leader catheter의 stylet) 을 넣는다

->mini port 제거

->노란색 central line insertion

->뚜껑을 모두 뺀 3-WAY 와 연결된 Suction line 연결

->N/S Insertion (N/S 종지에 10CC 담아서 올려놓는다)

->10CC sryinge 에 30cc insertion 한 후 suction 되는 양상으로 air 가 빠지는 것을 본다.

 

8.양쪽 시행

 

9.dressing

wet gz -> dry gz -> 3등분한 steri strip + smooth fcp 붙임

->tegarderm 작은 것을 절반 잘라서 그냥 붙인다.
