․ cordis AAA Stent (AB/IL)

․ surgi-loop (short)

․ U-tape (1/2 잘라서 사용)

․ suture boots (4ea사용)

․ c-arm 비닐

․ 4EB

․ Spoid 1ea

․ 10/30cc sry 각 2ea (dye용, heparin용)

․ horizon clip micro/small

․ 18g medicut 2ea

․ nelatone 6번 2ea (3등분으로 해서 slider 만들기)

․ 3way (cap용으로 사용)

․ Medress 9*10 2ea

 

6.suture

․ 5/0 prolene (8710) 4ea

․ Safil 2/0 or Vicryl 2/0 1ea

․ Safil 4/0 1ea

․ Steri-strip 2ea

 

7.약물

․ N/S 1L Pack (heparin 1cc 준비)

․ visi-sence(dye) 320 ml (1:1 mix 해서 사용)

․ surgicel 5*35cm

․ prn) beriplast 1ml

 

 

 

 

 

1.Drap

Universal drap + utility + 3M

Large utility 는 C-arm monitor에 붙일때 사용

 

 

2.Incision&approach

marker design

→15mess incision - Common Femoral Artery (Lt,Rt)

→bovie + Debakey dissection

→mastoid. vein ret, army retraction

→Femoral artery 나오면 Rt angle이나 mixter로 nylon tape sling 을 apply

→small branch들은 micro, small clip으로 cliping

 

 

 

3.Purs-string suture

FA 5/0 Prolene (F0-BB)

→6번 slider + mosquito

→5/0 prolene 2부위 purs-string

(Femoral artery 의 직경이 작은 경우 6-0 prolene sutrue)

 

 

 

4.Angio set in

(c-arm setting)

무영등 off, 방 불 c-arm을 등지고 off

 

 

18g medicut puncture

→guide wire 짧은 것 (6Fr introducer sheat 내에 있는 guide wire)

→Introducer sheath

→terumo guide wire(0.35-260 curved)

→Sheath내관 out

→heparin sry로 통과

→tower clip으로 sheat고정

→1)length를 위해 pig tail insertion 

→dye extension 연결해서 renal arteries 의 origin에 위치까지 확인

2)davis

 

1)terumo in - pigtail - terumo out - dye line 연결 (+tape)

2)terumo in - davis -terumo out - stiff wire(lunder quist) - stent 의 sheath in - sheath의 내관 out - davis out - stent insertion

 

stent + sheath (AB는 일체형) 하고 난 후에 stent 의 sheath 에 Heparin solution을 flushing

stent의 sheath가 들어가고 난 후에 3-way의 뚜껑을 막은후 guide wire 나 온 후 wire 제거

stent가 들어가고 난 후에 확실히 stent의 밀착을 위해

guide wire 넣고 - Reliant balooning cath insertion 하여 dye 30cc srynge로 balooning
