1.General anesthesia, supine position, 16Fr foley insertion 15cc ballooning 

 

2. Low midline incision 

20mess -> Short allis 2ea 잡고 opening

ret: baby richardson 

 

3.split the rectus muscle in the midline

sitck (kelly or spong clamp) 으로 bladder를 누르며 urethra 에 넣은 foely 만져서 

prostate의 apex에 위치시키면서 prostate쪽을 lg bovie tip으로 박리해간다. 이때, Lg allis사용 

 

4.Retzius space로 들어가 retroperitoneal space마련 -> omni retractor applied

Endopelvis fascia opened 

 

5.L/N dissection 

mixter / Rt angle -> surgi clip L/M 

 

6.Prostate apex의 lateral side를 dissection

Puboprostatic ligament & dorsal vein complex 확인

 

7.Dorsal vein complex ligation 

-head lamp 밖에서 cir. RN 이 씌여드리기 (환자의 왼편)

-asisst : stick (kelly or cvd sponge clamp)

-ligation with 1/0 vicryl (직침) distally and proximally 

-division & temporary bleeding control for oozing

-periurethral redundant tissue dissection 하면서 vessel은 hemoclip m/L 로 Clipping 

 

9.Urethral division 

prostate와의 이행부위에서 10-2시 사이를 bovie로 division 

Foely urethral catheter를 꺼내 kelly clamp로 잡고 catheter를 cutting 

completion of the division of the urethra : 15mess (lg handle)

urethral chateter 를 위로 당기면서 apex를 retraction 하여 recto-urethralis muscle을 divison : hemoclip -> metz

 

10.bladder neck dissection & transection of the bladder neck 

 

11.proximal prostate & seminal vesicle dissection 

 

 

 

 

12.rectum의 anterior wall과 denonviller's fascia사이로 dissection 하여 plane을 형성

bladder neck -> apex 방향

dissection & ligation of vas & seminal vesicle dissection 

Ligation & division of prostatic pedicle, both 

 

 

 

 

 

13.NVB(Neurovascular Bundle) 은 양측 보존함

*NVB : Sexual function and continenve 

 

14.4/0 Vicryl 로 baldder의 epithelial surface를 eversion 시킴

 

15.careful hemostasis : dorsal vein complex 주위, 양쪽 prostatic pedicle 주위 및 seminal vesicle의 lateral side의 vessel ligation 한 부분 확인

 

16.Urethrovesical anastomosis 

2/0 J type needle 로 stich through full thickness urethra wall out-in / in-out fashin : 1,3,5,7,9,11시

 

이미 urethra 에 tagging 해 놓은 suture material 2/0 vicryl 을 in-out 또는 out-in dmfh bladde neck에 방향을 맞추어 suture 해서 tagging

 

17.Distal urethral-bladder neck anastomosis : 16Fr foley catheter 삽입

 

18.JP drain insertion to the area of anastomosis *2

 

19.Rectus muscle, fascia repair with continous and interrupt suture with 3/0 black silk

 

20.subcutaenous repair with 3/0 vicryl, skin stapler
